This document is also called “certificate of impeccable conduct” and certificate of criminal record (zaświadczenie o niekaralności), as it certifies that an individual has no criminal record during his/her stay in Poland.
In essence, this document is an extract from the Polish National Criminal Register (KRK).

To issue a certificate of criminal record you will need: passport, correctly filled out application with a request for information about the person, as well as a receipt for payment of stamp duty.

IS IT POSSIBLE TO OBTAIN A CRIMINAL RECORD CERTIFICATE ONLINE?
It is possible to obtain a criminal record certificate online for foreigners who have a verified profile on the state platform ePUAP. It is called Profil Zaufany.
If you have one, you need to register on the website ekrk.ms.gov.pl (this is the official website of the National Criminal Registry). After registration, you need to send a request for a certificate on behalf of your verified profile confirming your identity. Payment of the stamp duty can be made there, on the website, through the eKRK system.
